It only happens when FF starts, safe mode or not. I also tried
disabling all my extensions, and that doesn't help either. Even when I
take out the .wav file it turns up again a couple of days later.
Actually, now that I think of it, I just have a new batch of MS
updates, and after a period of blessed silence the problem has started
again. Could they have anything to do with it? How would a MS update
be activated when Firefox starts???
No idea, sorry.
You could uninstall FF and see it it happens and then reinstall it.
