Are you rebuilding your database often? Entourage and the Apple Address Book sync to a database called the truth. If the truth gets messed up, it will just sync back the same bad data to your applications. You might want to turn off Sync Services and have clean up that application as your primary source. Delete the associated files and then turn Sync Services back on.
