Two parts to your question. If I'm not mistaken, the JAWS installation process gives you the option of loading or not loading JAWS at startup. If it doesn't, maybe you could post again. The checkbox is in the JAWS options dialog, but how you get there depends on whether JAWS is or isn't in your system tray.
Yes, control-alt-j is a good hotkey to open JAWS manually. However, you'll need to set it yourself.
