My experience with ITK 2.6.2 was that it is severely broken. Even though
going back to your previous ITK version probably won't fix your problem,
putting 2.6.2 in the mix is only going to make matters worse.

Basically the only reason you should get this error is the plugin is
inactive for some reason. Unfortunately I don't remember the exact
reasons why that would happen, and even more unfortunately for you I
can't really do much for 3.01b7.
I really recommend you upgrade to v4 as soon as is practicable, I
can give you much better support for that.
