The FAQ on the Market about refunds says the user is subject to the developer's refund policies and may not grant a refund. I would suggest no refunds and now that we have 4000 characters for description to state so.

Good question that. If a user asks for a refund after the 15 minutes has
expired, I can't see any way on checkout to refund *and* invalidate the
user's app access.

If I refund it, will they continue to get access and usage of the app
forever but now without having cost them anything?
