Rocky, We are getting this error with mid-tier 7.0.0 patch 1 on windows 2003. ARSystem is also 7.0.0 patch 1 on windows 2003.
We have pushed this as a bug to Remedy through our support partner and have been told that it is fixed in 7.0.1, although I haven't seen any release notes yet. There is a bug, SW00252963 on remedy's site which appears to be relevant to the 9388 problem we are getting (9093 appearing in mid-tier logs immediately before 9388 error). This is in QA status. The problem is you go to a mid-tier form and and the system prompts for a login. When you login the mid-tier page comes up normally. Now you click on the "X" button on the browser to close the whole browser. Now going to the same mid-tier page it prompts for a login again, but now you get an authentication failed error. Clearing cookies and cache does not fix this. If you release the user with the admin tool you can login. I have added workflow that does a run process and executes a javascript logout on window close; this seems to correct the issue, but people will be required to login every time. Any help on this will be appreciated.
