I want to start by saying that we have a big push right now to complete the Asciidoctor user manual.

For the HTML output, all that does is set the lang attribute on the <html> element right now. To get the the labels to switch over, you need to set them individually. -a lang=it -a caution-caption=Attenzione ... This is explained in detail in http://asciidoctor.org/docs/user-manual/#customizing-built-in-labels Assigning these attributes manually is necessary until https://github.com/asciidoctor/asciidoctor/issues/1129 is resolved.
