Since the days of yore, I have only ever seen the linking done in the
XLR, as this stops the "cold" (Pin 3) from becoming an antenna for
interference, and good amplifiers will expect this to be grounded for
unbalanced inputs.

On the "More Simple" diagram you have it almost right, however I am
attempting to wrap my head around your setup.

I shall assume you have a stereo pair you wish to connect to a pair of
active monitors.

Each RCA connector has one core (the pin)and a sleeve (the collar)
usually these come as pairs in a fig8 arrangement of cable (this is
good, as it allows for easy splitting)

from each channel take the core and wire it to pin 2 (hot) in the XLR,
and the sleeve and wire it to pin 1 (Ground) then link 1 and 3
together.

Now, if you have a mono setup with one active monitor (which is what
you appear to have from your drawings) this is the quick and dirty way.

Take both the cores and twist them together, and both the sleeves,
solder the cores to 2, the sleeves to 1 and then link 1 and 3 again.
This will then give you a mono output from a stereo pair.

I am not sure where you figured in the ground from your SPDIF, but it
really isn't needed.
