Links in the Index: I do not know of any way (using OOo's built-in
tools) to make items in the book's index link to the corresponding
pages in the book. I think there is an enhancement request to add this
feature, but I don't know the number.
Links in the Contents: I did not set up the Contents to link to the
headings, because that file was originally created only for the
purpose of printing. Sorry!
I plan to make a book from the full-size pages and it will have the
hyperlinks in the Contents. This has been on my to-do list for a long
time. I will try to find time this weekend to at least make the links
active in the existing file and upload it.
Meanwhile, you can change the Contents page yourself to have active
links: right-click in the Contents, choose Edit, then go to the
Entries tab and use the tools there to make active hyperlinks (the
chapter on Tables of Contents has more info).
Another strange thing is that the page numbers in the contents do not
correspond with the page numbers in the book. Has it to do with the
fact that the book has a format of 6 X 9 and my computer is set to A4
page layout? Trying to make the page setup to A4 format does not
solve the problem and makes it even worse.
It sounds like there is something wrong with the file. Try
right-clicking in the Contents and choosing Update and see if that
fixes the page numbers. Let me know if that fixes the problem.

Why not take the individual chapter contents (.odt files) and simply
reset them using new A4 page styles and reformat (update) everything?
That should almost be a no-brainer.
