A "fix to the pagination"? Just right-click and choose Update ToC and all should fix itself (topics as well as pagination). Many people -- including me -- often forget to do that, which is why the ToCs are often out of synch with the chapter.

There's also something weird that happened with the numbered list style that I'm not sure about. When the pages re-flowed because of the new material, the numbers are now outside of the page border.

I've seen some weird effects in some files recently, but not that particular one.

Gary, I tried your suggestion for rejecting the note, unfortunately it
didn't work.  It set the note back into an "undeleted" state, and when
I tried to delete it again it just put the X through it as before.

You have to turn off Track Changes first before that will work.
