Some other thoughts (again, probably things you've already considered): My understanding is that "Cota/Cotta" is a name that's most commonly found on the island of Terceira, and that on Terceira the densest distribution is around Doze Ribeiras, Santa Barbara and Altares. Maybe Velma was an Americanization of Vilemina/ Velmina (I'm not sure of the spelling-- I'll ask my mom if it's something you'd like to know for sure).
