I checked out a clean copy of boost and moved the ntp stuff over to
it.  I then found the missing piece: the dependencies in doc/Jamfile.v2:

      <dependency>../libs/ntp/doc//autodoc.xml
      <dependency>../libs/ntp/doc//ntp
      <implicit-dependency>../libs/ntp/doc//ntp

After adding that, everything works.

It still doesn't explain why I was picking up xpressive headers.  I
suspect there was some odd state in the old directory, and that
should be investigated and fixed.

But at least I'm getting documentation now.
