If you haven't seen this already, the diagram below could be helpful showing how the water travels from pump into the group. (It's for the IV but I don't think this part changed since the II - correct me if I'm wrong anyone!). Note it goes through a heat-exchanging pipe in the steam boiler first (to preheat) before getting into the brew boiler. If I were to guess however, I'd say a clog would likely happen somewhere in the e61 grouphead itself since there are some pretty small channels and orifices there... but it could also be anywhere... Anyone else who has experienced a similar situation might shed more light on this.
Another thing that won't hurt to verify is, look at the drain tube coming from the OPV back into the water tank. if there is significant resistance on the brew outlet, you might see all the water come out of the drain tube when you turn on the brew cycle.
