The problem is most health insurance companies do not answer to those for whom they provide coverage, nor for any company with whom there is a 'group' plan. No, most health insurance companies answer to their shareholders.
And shareholders typically demand a profit. That right these is the problem. Healthcare decisions in insurance companies are not based on what is best for the patient, rather, what is best for the shareholders, and rarely, if ever, will they both benefit at the same time.
