I've only done this with RealPlayer streaming - I don't think Flash will do the trick, even communicator, for live streaming. The video quality to file size ratio isn't there (let the flames commence).
If you used RealPlayer you could use RealOne and use CF to place the ads in the HTML space. Or use SMIL and generate the SMIL dynamically.
