I think the fear is from his first experiences.  I don't think he has gotten completely over them yet.  I don't think that you have to pick him up everytime you pass someone.  I think that you should put the muzzle on if you can find one other wise everything else is the same.  Just stay calm, he can since your feelings so if you are a nervous wreck he will be too.  Put on his leash take him out and keep his leash short...so that he has to stay right beside you.  When you pass someone and he begins to react immediatly turn the other way and walk away from them , keep calm don't jerk him away or scream at him or show any emotion at all just abruptly turn away from what ever stimulus is causing his reaction.  I think that in the end he will see that you are the boss and you will keep him away from scary situations.  As his behavior improves and he doesn't react when he sees another dog or person allow him to get a lttle closer each time until he reacts (then turn away) or he is comfortable being near someone else.  Hopefully this helps good luck
