Check the internet for SpamPal (free!). Kills most of the spam. You can also try Mailwasher. I am running: Spampal, McAfee, a firewall of course, several anti-add software like spysweeper+spybot+ ad-aware, a filter that kills any spam at non-legal addresses of my domain (try sending an email to [EMAIL PROTECTED]), and of course I don't use Outlook.
