I had a 13 week old pup present with these symptoms.  It was Eosinophilic Myostitus 
(sp?).  I certainly hope this is not what Jan's friends dog has, it was all down hill 
from there.  Nicky was never able to fight off any infection or bug that was going 
around.  The things that most normal healthy dogs are able to combat naturally.. It 
was a year of endless ear and eye infections.  At one time she caught her toe nail 
somewhere and we had not noticed, the next morning she had almost chewed off her 
entire toe, you can imagine the problems that followed that incident.  Nicky lived 
entirely on antibiotics, for 12 months.  Eventually after numerous tests and visits to 
various vet schools she was euthanised just before her 1st birthday.  It was a 
dreadful year,  it was going back a while (about 10 years) and while we were in UK, so 
my exact recollections are faded.  However, I DO remember the name of it, and the way 
it started out.
Don't know if this is any help, mention it to the vet, see if it helps.
