It would depend on the buyer. If they were attached to the dog and wanted to
keep it, then I'd give them the choice of a total refund or another pup and
let them keep the first dog.

I wouldn't let them rehome it and I wouldn't rehome it myself. Why let
someone else get attached to the dog when it will probably not live long?
I'd take it back and keep it myself and give them a refund or another puppy.

If I didn't have another prospect, I'd refund their money.

Another thing that I would do is take a look at the breeding that produced
this puppy. Showing MVD at 6 months is ominous. I'd have the parents checked
by a cardiologist, if this had not already been done. Then I'd take a look
at the line breeding or any common ancestors if the parents are cleared. The
remaining thing I'd think about is whether it would be wise to spay/neuter
the parents.
