I don't think that the idea is to not assess, but whether to use a separate assessment form for everything. Much of the time, you figure out there's a problem from at least your 5 day assessment, and hopefully the skilled nursing notes (charting, forms, checkboxes, etc.) have the information. A B&B assessment often does need a separate form to get the needed info, but everything else is pretty much spelled out. I once worked for an IHS faciity that had it's admission nursing assessment almost box for box the questions from the MDS for nursing. Then the skilled nursing forms had boxes to capture the needed assessment info, and you had your charting and initial information in front of you. Best charting I've seen in a long time!
