As MDS Cord., I do pressure, discus, inital fall upon admission, s/r eval, b/b. Any falls thereafter, I don't do them, update all the discus, update pressure/skin assessments q92d or if assessment is due before hand, s/r q qtr. I don't do dinning, activities etc, when would you have time, it takes me 3-6hrs to type good raps and my c/p's are extensive takes a while. I am in a 122 bed facility with about 70 medicare & long term mix. I make sure q medication has a diagnosis, and obtain one from the dr..., it does seem to be on top of the list for the floor nurses.
