We do
Elopement Risk, Pressure Ulcer Risk, B&B Assessment, and a pre-dmission Fall
Assessment. Then, "Notification of Risk for: Falls, Skin, Elopement if
assessment reveals that they are at risk. We also do a Pain Assessment as
needed. All the admit assessments are usually done by our DON, with the ongoing
and new assessments done by either staff nurses or MDS nurse. My last facility
also did a Safe Smoking Assessment. We don't re-do all the assessments with
every MDS, only on a PRN basis. This may change as orginally we were planning on
doing them with all OBRA assessments.

Hi all, I'm in a stun mode right now. I have another question. How many , and what kind, of assessments is everyone doing "during" an assessment period. We have : braden skin assessment, fall , dining, foot, education, spiritual (activities does this one, all others are nursing), pain, mini mental, side rail, and elopement. We are looking to consolidate and reduce!Any thoughts would be greatly appreciated!Thanks, Nancy
