We do Braden, falls risk assessment, DISKUS,Self administration of medication,Restraint review, Bowel and Bladder, and Pain assessments quarterly with each review. We do audits monthly on Dining. (Care Coordinators) We have a skin prevention protocol in place for res. at high risk and the c.n.a.’s and the L.N. are responsible to document weekly skin checks. We have the nurses doing diabetic foot care/nail care weekly and documenting on the MAR.� We only do mini mentals as indicated.� We are hoping to start doing them on all admissions. �Our LSW”s do these.� We don’t do side rails assessments anymore cuz ours are all � inch rails.� If they use full rails then we include that in our restraint assessment.
