Depending on how much space was available, and whether other diagnoses were more significant, I may include the constipation at I-3. I would not code it in H unless I had documented c/o or symptoms of constipation in the 14 day observation period.
 
For Nephrostomy tube, instructions for indwelling catheter specify "maintained within the bladder" so nephrostomy wouldn't be coded there. It would be coded as an ostomy ("any type of excretory ostomy of the ...genitourinary tract")  Ostomy care would be coded at section P, so if you are doing any sort of site care, code it in P as well.

Hi to all and Merry Chirstams and a Happy New Year
 When you have a DX of Diabetic Insipidus with Chr consptiation with tx but no constipation problem seen would still code it as constipation like you would with the DX of dysphagia with diet tx or not?
Also where  do you code a nephro tube in section p and h , neither , both or just p and just h.
