I believe what you are looking for is whether to discharge track with return anticipated or not. If the resident is discharged to the hospital and you expect them to return, you would discharge track with a return anticipated. While it is optional, (also depending on your state requirements), if the resident expires, you may also send a discharge tracking sheet with return not anticipated and use death as the reason.
   Of course, if it was a new admission and they went out to the hospital prior to their initial assessment, you would have discharged tracked as prior to the initial assessment. Once again, it would be optional as to whether you did another tracking form to discharge track with no return anticipated.
