One step further, if you feel the reports you are receiving are incorrect you need to go back and assess the MDS for accuracy. Maybe there are some data entry errors or something was present during the assessment reference window that is not present now that you forgot about. These are just general comments, but when helping facilities find out the differences these are some of the concerns I have found.

Maybe I can help with this. The Facility Quality Indicator Profile does not include data from the initial admission assessment, but the Resident Level Quality Indicator Summary includes all MDS assessments including the initial admission assessment, therefore they can be different.

I am having problems with the the quality indicator report also. I have
placed 2 calls into CMS for clarification and have not gotten any response.
What I have found is that the numbers on the Facility Quality Indicator
Profile do not match what is on the Resident Level Quality Indicator
Summary. I dont know what is going on. I hope I hear back from some one
soon.
