Although I'm not sure of the details that seem off on your reports, one possibility is that the Facility Quality Indicator Profile excludes Admission assessments, whereas the Resident Level Summary does not. If you check the link above you can see a table indicating this. This plays in to how individual indicators are reflected on the reports. Something to consider.
