What you need to show them is that all disciplines have been assessing the
resident for the appropriate observation period.  Your ARD is the most
important date, all of the assessment windows look back from there.  So, you
would need to do a head to toe skin assessment sometime in the 7 days prior
to the ARD including determining if anything changed during those 7 days.
The mood would be looking back 30 days from the ARD.  If your disciplines
give you worksheets or assessments, it needs to show that they were also
assessing the same assessment period.  Maybe placing the ARD on each of the
assessment papers and showing which days you were looking at would help.  My
therapy department finds it much easier if I put the begining and end date
of the observation window on the paper for them.  That way they are sure
which days we are assessing.
