The customer that gets me to outsource work to those places I mentioned (I admit I wouldn't if it was my choice) insists that I use elance.com and only hire people who have straight 5's plus good comments in their feedback ratings. It does work. Of course, some of them turn out to be useless, but some don't.
To be honest, some of this is just basic management. If you want code with certain characteristics, you have to document the standards you require, have the confidence to not hire anyone who doesn't show evidence of being able to adhere to them, tell the developers what the consequences will be (eg not getting paid) if they don't adhere to them, examine the deliverables early & often to ensure you don't get any nasty surprises (this means going to a lot of trouble to define deliverables that you *can* examine early on), and follow through on any threats/promises. But this is true of managing developers wherever they are - it's harder work than coding, and less fun too. You won't get results by just saying "I want such & such an application" and then just sitting back and waiting for it. Allowing someone to produce so much bad work that it takes 4-5 months to put it right is pathetic, really.
