Well, if the problem is word inserting junk, then the behavior should be different for native files and imported ones.
For native files: * If there is a document title- use it. Otherwise, use the filename.
For imported files: * Default to displaying the filename.
This is assuming of course that the people complaining here set the title on native documents, not on the word stuff.
