For two of the three users, the problem eventually went away by itself. In
the last case, we found a piece of Chinese-language spam while looking at
the account through OWA. After that was deleted, the Inbox started updating
again. 

Since we did a lot of cache-emptying during this process, I am now wondering
if having sizable subfolders in the Inbox could possibly stop/slow down the
updating of new messages showing up in the Inbox while Entourage re-caches?

If that is the case, is it safe to move the subfolders to another location
on the Exchange account while the re-caching is in progress? If it is
unsafe, is there a better way to make Entourage update the Inbox
periodically? Quitting and restarting the app seems to work some of the
time, but not consistently.
