IMHO rule 2) is incorrect.  It should be:

   2)  when lens is on or off camera.

When the lens is ON the camera, its benefits are obvious.  When the
lens is OFF the camera there should be caps at BOTH ends of the lens
and the hood should be reversed on the lens for two reasons:
   The hood remains handy so that it is more likely to be used when
      the lens is mounted on the camera.
   The reversed hood provides some degree of shock protection for
      your lens.
