I have hoods for all my lenses as well, but there are some
situations when I don't use them:

* In macro photography when the hood would prevent me from
getting the lens close enough (with a little extension 
the 100/2.8 USM macro will focus inside its hood!).

* When I can't position a flash so it wouldn't be
blocked by the hood (also mainly in macro shots).

* When I want to use filters that won't fit with the hood on.

* When using a polarizer in fast situations with certain lenses:
switching from horizontal to vertical takes forever if I have
to remove the hood, rotate the filter, reinsert the hood...

* Sometimes when I want to avoid looking professional
or attracting attention (or indeed intimidating people).

* Sometimes also simply because there isn't enough time
to mount the hood. (Quite rare though... if at all possible 
I carry the camera with lens hood already mounted.)

Still, I probably use hood for over 99% of the time.
