I have placed my response through out your questions. When I first was
interested in the Canon system I
was able to barrow an EOS3 with a 550ex strobe, 28-70 2.8 and a 200 1.8. I
was hooked.

Where did you get that information? I can barely understand that it
might be popular with fashion photographers (they'll try and/or do
anything to be different) but I cannot figure out where a wedding
photographer would find a use for any 200mm lens.
Dennis Reggie is a world famous wedding photographer (did all the Kennedy
weddings-some of those images made Life magazine)
he said that he would not have wedding kit with out the 200 1.8 included.
As for my self (I also shoot weddings) I love the lens
for balcony shots during the wedding. with a 2x extender you now have a 400
3.6 and you can get right down on the couple-the brides
love it. I also use the lens for full length bridals, portraits of children
(full length), candid shots of people at wedding receptions, etc.
I worked as an assistant to a wedding photographer back when I was in
college (longer ago that I'd like to remember) and more than 90% of our
shots were with the slightly wider than normal 90mm lens (on a 6x7
Koni-Omega). We'd shoot a few portraits with a 150mm on a Hasselblad,
but never anything longer.
I know there are favorite lenses that most of the work is done with at an
event and for us that is the 28~70 2.8 , but we also shoot with the 15mm,
16-35 and the 70~200 and the 100 macro. And everyonce in a while the 300
2.8. Times have changed since the press camera days.
With the change to 35mm, I can't imagine using lenses longer than
85-100mm for anything. In addition, I really can't imagine investing the
money for a lens as expensive as the EF 200mm f/1.8 to be used for as few
shallow-depth-of-field shots as you might want to take in the average
wedding. A very bad business decision on the part of the wedding
photographer.
The purchase of this lens, although pricey, has paid for itself many times
over. It is a work horse.
In addition, it was really only the digital era that drove medium format
out of the wedding business and most wedding photographers that I know
are using 10D-20D-30D (NOT 1D or 1Ds) so the 200/1.8 becomes a 320/1.8,
even LESS useful for wedding photographer.
Sorry but most of the folks that I know in the business are not only using
those but the 1D and 1Ds and the MKII versions of both. I shoot atleast 200
images at every wedding with the 1Ds. I love the 200 on my 30D great for
tight face shots of the little ones.
