I use WinClam, which is an implementation of ClamAV for Windows all
the time. It works quite well. The only feature it's really missing is
the "real-time" scanning of files when they are accessed.

At the very least I've found it to be a useful second line of defense
as it can (and has) catch things other scanners miss.

IIRC there is an open-source antispyware tool for windows that can be work together with clamwin to do real-time scanning.
