I do a lot of handbell arrangements and have worked out a process of including the "bells needed" measure that appears above the first line of music. The basic process is to format the first measure of the piece with no key signature, forcing precautionary accidentals, suppressing the meter signature, bar lines and stems and the precautionary meter signature at the end of the system. When I am ready to format the page I place only that one measure on the first line. With meter and key signatures forced into the second measure (at the "real" beginning of the piece) the page then appears the way I want. Remember to renumber the measures so that the first measure of music (measure 2) appears as measure 1.
