By sheer coincidence, last night I decided to make a minor contribution
to FLTK by providing an updated FAQ entry about this, although it does
not answer your last question.

Go to the Articles&FAQs tab on the web site, and you should be able to
see the old and new versions of "What are the Versions of FLTK" if you
search the FAQs.

New: http://www.fltk.org/articles.php?L825+I0+TFAQ+M20+P1+Q
Old: http://www.fltk.org/articles.php?L366+I0+TFAQ+M20+P1+Q

I would welcome any constructive comments, or points that you feel
should be added or clarified.
