I have just completed two nightmare installs of OS X in the last week. One was to a B&W G3 and the other to an old Wallstreet. I made several unsuccessful tries and went looking for information. I found an Apple TIL that talked about problems installing on systems that had both SCSI and ATA on board. It seems that it is necessary to have both ends of the SCSI chain terminated and this does not happen if you have nothing connected to the port. So you can either simply connect a terminator to the port, hook up a terminated device or in the case of the B&W G3 remove the SCSI card. In both of my cases this was the problem. What resulted in a successful install for both the B&W G3 and the Wallstreet was attaching an external SCSI device to the port and having the device properly terminated. So for both of my machines the trick that finally resulted in getting OS X to install was simply attaching a properly terminated Scanner to the SCSI port.
