I have good suggestion to Phillip. If you first time run the game, there can be only easy diffaculty, and if you beat the game, you will get another diffaculty, etc. If you complete game on insane, you can play another mode: A chalenge mode, or survivar mode, where you must killing many enemies, and stay in life as long as you can.
