With help from dhougal, I've been able to track down the issue.

Gears for Safari does not load correctly on case sensitive HFS file systems,
these are relatively rare but this is still a case we need to fix.

I've filed this as http://code.google.com/p/gears/issues/detail?id=766 which
we'll update when we've resolved the issue.
