I'm just downloaded and installed Gears for Safari on OS X and I am
experiencing the exact same problem as dhougal:
* gears.google.com says Gears is installed
* no Gears option in the Safari menu
* example apps (I tested Desktop shortcut app) do not work
* there is a green down-arrow icon in Google Reader, but clicking on
it fails (synchronizing failed)

My report:

* iMac: 2.4GHz Intel Core 2 Duo, 3GB SDRAM, OS X 10.5.5
* My console does not print any output upon starting Safari (from /
Applications/Safari.app/Contents/MacOS/Safari)
* ls -al /Library/Internet\ Plug-Ins/
