Having experienced working in Medicine prior to Medicare I've decided that the best system, after all, is to give the patient an account for what one thinks the service is worth and leaving them to bring Medicare into it.    As far as is possible we have ignored Medicare's rules and what it is prepared to refund the patient.

Good on yer Greg. For an old leftie, you've got more than most - courage and a clear idea of your value.

I wish you the absolute best of outcomes from the new venture - it's not just the staff who will need educating though.

As far as item numbers go, you could substitute a description of the service and the time taken for the item number and let the patient and Medicare haggle over the refund.
