I do all this very happily with a crappy 256/64 ADSL connection
at the surgery (with fixed IP, and using Best Practice), even if I
use a dial-up connection/VPN from outside. 

Obviously a faster ADSL plan will improve speeds especially for
remote printing of scripts etc, but my slower Internet connection
is still very workable. 

As you can see, I didn't avail myself of the B4H offer - it just didn't
seem necessary to me.
