It picks up registrations on app start-up (by detecting the launch and peeking in the launched app's bundle). If you launch an app, then start Growl, Growl won't know about the app unless the app is already registered from a previous launch.

This is the one case for keeping squelch mode. We could fix it by having Growl scan already-running apps on GHA's launch.
