I've also had this problem, but only when doing a LAN game. If I join one of our development servers with other people playing, and quit the game from there, hl2.exe doesn't show up in task manager. But if I do a LAN game to test something and then quit, I can't restart the game until I end-task hl2.exe.
I've also crashed a few times as I've been working on the code, but clicking the OK button on the crash dialog box didn't allow hl2.exe to stop. I still had to manually end-task it to get it to quit. This seems to be a fairly recent thing, as I don't remember this happening prior to this weekend. Also: EP1 engine mod.
