It looks like you need to add +ip <ip_address> to your hlds arguments. Be sure to use the internal IP, not the external IP. I had the same problem when I set up my first server.
All you really need to forward from the router to your server for your setup is UDP 27015. I played on your server for a few minutes, so that appears to be set up ok. Man, I haven't played that map in a while.
