Yes they would all have to be used up. Do you have the SMTP service setup to All Unasigned in the SMTP Virtual server properties or are you specifying the ip address. You should specify the IP address instead of using all Unassigned. this will probably correct the problem.
To see the outbound ports being used use netstat -n. This will show all outbound connections on the server. Pipe the output to a text file so you can analyze it / Count the number of current outbound connections for a specific server ip address. I didn't notice what port numbers were even being used. Is it safe to assume that ports 1024 to 5000 would be used before this would have any impact?
