I'm going to make a suggestion to all this. We switched from Exchange to
Imail.
Spam filtering being one of the decision qualifications. After fighting the
same battles you're facing we opted
for Declude Junk mail Pro (Imail addin). Though tough to convince the powers
to be to spend more money, after
getting it all tuned (about 2 weeks), management has agreed it was a great
choice and worth it.

Your talking about subject modification...declude does it. FYI we rate each
email
0-9, not spam
10-35 possible spam, modify subject
36-90 spam, put in spam box
90-300 autodelete

Users within the domain(s) are authenticated thus automatically white
listed.
Users can use the web based email / address book to whitelist who they want.
If you add someone in
the address book, they're whitelisted.
