Not that this addresses your question about subject modification but couldn't your users already use client rules to sort incoming mail by using the X-Headers that IMail anti-spam already appends? In this case you would not take any action on the incoming mail marked as spam but the X-headers would still be appended to allow your POP clients to filter as they please at the mail client level.
