Can you isolate it to a single file in the queue that the server is hanging on? You might try moving all of the queued files to another directory and moving them back a few at a time to isolate a potential cause.
Once the smtp service is stopped and restarted, everything in the queue gets sent out with no problems. Would looking at the syslog and seeing what the first message is that hangs help?
